Percutaneous gastrostomy with computed tomography guidance.

Abstract

Purpose: To describe the procedure of percutaneous
gastrostomy with the guidance of computed tomography
and our experience.
Materials-Method: CT guided gastrostomy catheters
were placed in 5 patients with a primary cancer of the
oropharynx. Three patients had a Wills-Oglesby 14F
catheter placed in the stomach and in two patients a
balloon 18F catheter was placed with the Seldinger
technique after adequate dilatations of the stomach.
Four patients had the procedure performed before
surgery and one post-operatively prior to radiotherapy.
Follow up of the patients was scheduled every two
weeks for as long as the catheters remained.
Results: All 5 gastrostomy catheters were successfully
placed and no procedure related or late complications
were reported.
Conclusion: Percutaneous gastrostomy with the
guidance of computed tomography is feasible, well
tolerated by the patients and is an effective way of
gastrointestinal feeding support.

Σκοπός: Να περιγράψουμε την τεχνική της δια-
δερμικής γαστροστομίας υπό αξονικό τομογράφο
και την εμπειρία του τμήματος μας.
Υλικό-Μέθοδος: Πραγματοποιήθηκαν 5 δια-
δερμικές γαστροστομίες στον αξονικό τομογράφο
σε 5 ασθενείς με καρκίνο του οροφάρυγγα. Σε
4 ασθενείς οι καθετήρες τοποθετήθηκαν προεγ-
χειρητικά και σε έναν μετεγχειρητικά πριν από
τις συνεδρίες ακτινοβολίας. Τοποθετήθηκαν 3
καθετήρες Wills-Ogelsby 14F και 2 καθετήρες
μπαλόνι 18F στον αξονικό τομογράφο με τη μέθο-
δο Seldinger μετά από τις κατάλληλες διαστολές
του στόμαχου. Οι ασθενείς παρακολουθήθηκαν
στο τμήμα κάθε δύο βδομάδες για όσο διάστημα
παρέμειναν οι καθετήρες.
Αποτελέσματα: Η τοποθέτηση των καθετήρων
ήταν επιτυχής σε όλους τους ασθενείς χωρίς να
σημειωθούν πρώιμες ή όψιμες επιπλοκές.
Συμπεράσματα: Η διαδερμική τοποθέτηση κα-
θετήρων γαστροστομίας είναι εφικτή υπό την
καθοδήγηση του αξονικού τομογράφου, είναι
ανεκτή από τους ασθενείς και αποτελεσματική
μέθοδος γαστροεντερικής σίτισης.
